What to Bring and Be Prepared For

Driver License Services 

Please bring the following:

  • A completed Driver License Application. Applications are available at our office if you wish to complete yours in-person.
  • Valid, original, and unexpired forms of identification, such as a birth certificate or US Passport.  View the complete list of acceptable identification documents here.
  • If you are getting a Real ID, bring two (2) pieces of mail with the applicant’s name and current address. Must be from official sources (ex. bank, utility, government, school, or employer). No packaging labels or invoices.

Applicant’s first, middle, and last name must match across all documents. Bring legal proof of name change if your name does not match. An official court order is valid proof of a full name change. A marriage certificate or divorce decree is valid proof of a last name change. To make your visit easier, change your name with the Social Security Administration first before your visit.

If you wish to designate a ‘VETERAN’ status on your Travel ID/Driver License, bring in your DD214 form (a copy is acceptable) or a Veteran Affairs card (must be original).

Learner’s Permit Issuance 

Please bring proof showing the applicant’s name upon passing the written test. Applicants under the age of 18 must be accompanied by a parent or legal guardian. In cases of divorced parents with joint custody, both parents need to be present. If one parent cannot be present, they must have a notarized signature on the back of the Driver License Application (where indicated) which can be brought to the appointment. 

If accompanied by someone other than the parent or legal guardian, the “Legal Guardian Certificate” section (located on the second page) of the Driver License Application must be completed with a notarized signature by the parent/legal guardian prior to the appointment. 

Driver License Reinstatement

Verification must be done in person. Requirements may vary.

Title & Registration Services

Please bring in original and physical documents. Altered documents are not accepted. Your vehicle will need an emissions test done prior to your visit if it is over 5 years old. All owners must be present to sign documents. An original registration is needed if there is a lienholder on your vehicle from out-of-state. If it is a leased vehicle, an original Power of Attorney form from the leasing company is needed. Same requirements apply for out-of-state vehicles. 

Vehicle Permits 

Qualifications for duration of usage (3, 30, or 90 days) are contingent upon documents submitted.

Special Plates

Certain special plates have eligibility requirements. Please ask us about what you need to provide, if needed.

Disability Placard

Please bring in a completed and original Disability Placard Application. The medical portion requires an authorized medical professional in the state of Arizona to complete on your behalf. Out-of-state disability placard applications are not valid and do not carry over.

Statewide,temporary credentials are provided for all services upon completion. Official credentials are centrally printed and mailed by ADOT. Please contact ADOT at (602) 255-0072 if you have not received your official documents within 30 days.

Airpark Motor Vehicle Services is an Authorized Third-Party provider for the ADOT Motor Vehicle Division. Convenience fees apply to all transactions. There is an additional processing fee for all credit or debit card transactions. All major cards are accepted.
