Disability License Plates and Placards

To legally park in designated spaces for disabled drivers, a vehicle must clearly display a disability plate or placard, indicating that it is operated by or transporting a person with a disability.

Whether you need assistance with an initial application or renewal, our goal is to make the process as quick and easy as possible.

Apply for a Disability Plate or Placard:

  • Complete the top section of the Arizona Disability or Hearing Impaired Plate/Placard Application and ask an Arizona-licensed healthcare provider to complete the bottom section on your behalf.
  • Bring the application to our office for processing. You’ll receive a temporary placard to use until  until your official placard arrives in the mail.
  • Temporary placards are valid for 6 months. Permanent placards are valid for as long as you continue to qualify for it.

Disability License Plate
Disability Plates are special license plates that signify the vehicle owner’s disability status. To qualify, you must be the owner or leaseholder of your vehicle or an organization that own or lease vehicle(s) that primarily transports persons with physical disabilities.

Apply for a Disability Plate:

  • Individual Owners: Complete the top section of the Arizona Disability or Hearing Impaired Plate/Placard Application and ask an Arizona-licensed healthcare provider to complete the bottom section on your behalf.
  • Organizations: Complete the Organization Disability Placard/Plate Application
  • Bring the application to our office for processing. You’ll receive a temporary plate to display on our vehicle until your official plate arrives in the mail. Your disability plate will replace your existing license plate, and will be valid for the duration of your vehicle registration.

Absolutely! Certain special plate designs are available to be customized with a disability symbol, and you can certainly add the disability symbol to your personalized plate as one of the 7 characters allowed.

Temporary disability placards are valid for six (6) months. Permanent disability placards are valid for as long as the person qualifies. Disability license plates are valid for the duration of your vehicle registration.

Once your application is approved, we’ll issue you a valid temporary placard or temporary plate that can be used right away until your official placard or plate arrives in the mail.
